By virtually signing and submitting this form, you are consenting to the following: 

Permission to Treat/Transport
Church personnel have my permission to treat and transport my child/children in the event of a medical emergency. My child(ren) have my permission to participate in any walks or events planned as part of the RE program, provided my child(ren) is under adult supervision and that I am notified in advance when my child's/children's class will leave church grounds.

UUCSH Friend Status 
By registering my child/children in the RE program, I understand that I will be considered a Friend of UUCSH if I am not already a Member. I understand that as a Friend I am under no obligation to become a Member. I understand that as a Friend my personal contact information will be included in the church directory to facilitate communication with other parents. I understand that as a Friend, I will be canvassed, meaning I will be asked to make an annual pledge to UUCSH. While I understand that a pledge is not mandatory for my child/children to participate in the RE program, because the program does not charge an enrollment fee, my pledge to UUCSH helps defray the costs associated with the program.
